The WWYFL is a member of the American Youth Football League and the American Youth Football League Cheerleading and conducts all activities in accordance with the AYFL/AYFLC By-laws. My signature hereby acknowledges that if my child was affiliated with another AYFL/AYFLC team for the PREVIOUS season, I will disclose this information and seek a proper release from that program.

The WWYFL is an all volunteer, not-for-profit organization and it does not discriminate on the basis of sex, religion, race or handicap. By signature below, I, the responsible parent or guardian, acknowledge that there are risks associated with tackle football and cheerleading. In the event of injuries or illness resulting from participation, I hereby waive all claims against the organization, officers, coaches, sponsors or supervisors and hereby release and absolve WWYFL and the City of Weston from any and all liability. I likewise release from responsibility any person(s) transporting my child to or from activities. I understand that I am responsible for my child’s medical bills if injury occurs. I give my consent for medical treatment by the closest hospital, doctor or medical facility. My child is medically cleared for participation.
